2.   I acknowledge that the practitioner is not a licensed physician and does not practice allopathic medicine. Services do not include medical diagnosis, treatment, prescription of medications, surgery, radiation, or any conventional medical procedures.

3.   I understand that bioenergetic and informational medicine is considered complementary or alternative and may not be recognized or understood by all licensed medical professionals.

4.   I understand that no claims are made regarding the cure, prevention, or treatment of any disease or medical condition, and that results are not guaranteed.

5.   I acknowledge that the services provided are not a substitute for medical care, and that I am responsible for consulting a licensed medical professional regarding any health concerns.

6.   I confirm that I have disclosed all known medical conditions to the best of my knowledge and agree to inform the practitioner of any changes to my health status.

7.   I authorize Juneva Health and the attending practitioner to perform bioenergetic and biofeedback assessments within the scope of their wellness practice.

8.   I understand that Juneva Health reserves the right to decline, discontinue, or restrict services if information provided indicates that services may pose a risk to my health or the practitioner’s safety.

9.   I understand that appointment times are reserved specifically for me. I agree to the stated cancellation and late policies, including any applicable fees for missed or late appointments.

10.   I acknowledge that I participate in these services voluntarily and accept responsibility for my decisions related to participation.
